Crystal Clear Mental Health is located in Rapid City, across form The Monument. Our address is 550 N. 5th Street, Rapid City, SD, 57701.

Crystal Hollenbeck is a board-certified Advanced Registered Nurse Practitioner (APRN), certified in Psychiatric Mental Health (PMHNP-BC). She holds a Master of Science in Nursing from Mount Marty University and brings nearly two decades of direct patient care experience to her practice.

Our regular office hours are Monday through Thursday 8:00AM to 4:00PM and Friday 8:00AM to 2:00PM.

Initial consultations are comprehensive and typically run 60 to 90 minutes. Follow-up visits, including telehealth and standard check-ins, are between 20 and 45 minutes, based on complexity and if any therapeutic interventions are used. We take pride in never rushing our patients- if something important comes up during your visit, we take the time to address it properly.

Yes- we accept many major insurance plans for psychiatric and mental health services, including Blue Cross Blue Shield, HealthPartners, Avera, Sanford, and Cigna. Please contact your insurance company to verify your specific plan before scheduling.

We accept all major debit and credit cards. Payment is due at time of service. We do not accept personal checks.

Yes- telehealth appointments are available for follow-up visits. Please note that initial psychiatric evaluations are typically conducted in person. Contact us to discuss which appointment format is right for your specific situation.

Medication refills require a follow-up appointment. We do not process refill requests outside of scheduled visits. This policy ensures your medications are reviewed regularly and that your treatment plan remains appropriate and safe.

Crystal Clear Mental Health does not provide crisis intervention or emergency mental health services, and we do not have admitting privileges to any inpatient facility or hospital. If you or someone you know is experiencing a mental health emergency, please call 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room immediately.
